The Maintenance Plan Sign-Up Form: Card Captured Before the Plumber's First Visit
A maintenance plan that starts with a phone call, a handwritten note and a promise to “get your card next time” is a plan that leaks. Members forget, the note gets lost, and the first visit ends with the tech asking for payment at the door. A sign-up form fixes that by collecting everything the office needs, including the card, before anyone drives out.
From the Doorstep to the Form
In Selling a Flush Plan at the End of a Plumbing Repair Call, the goal was to get a yes before leaving the house. The sign-up form is how that yes becomes a real member with a card on file and a plan on the house.
PlumbingBossPro includes a sign-up form you can share by link: on your website, in a text after a repair call, in an email to past customers or on a door hanger with a short link. The customer fills it in on their phone or computer, and their card is captured as part of the sign-up.
What the Form Collects
The form gathers what the office needs to set up a member without a follow-up call:
- Name, phone and email, so reminders by text and email reach them.
- Service address, which may not be the billing address for a landlord or someone signing up a parent's house.
- The service they want, such as a yearly water heater flush.
- Their card, captured at sign-up.
Keep the form short. Every extra question is a reason to close the tab. You can collect the water heater details on the first visit, when the tech is standing at the rating plate.
Form Responses and One-Click Convert
Every submission lands in Form Responses in the office. Someone reviews it, checks that the address is in your service area and not a duplicate of an existing client, and then converts it into a client with one click.
That click creates the client with the address and the card on file. From there the office sets the yearly flush plan on the house, at 365 days between visits, and either schedules the first visit right away or lets it come due.
Checking for duplicates matters. If the customer already exists from a past repair, attach the plan to that record instead of creating a second one. Two records for one house can mean two pins on the Waiting List and a confusing history.
Why Capture the Card Up Front
Collecting the card at sign-up changes the whole plan:
- The first visit is paid at completion. When the tech taps Complete, the invoice is created and the card on file can be charged.
- Every future visit works the same way. No more knocking on doors for payment or mailing invoices for a flush.
- Commitment is real. A customer who has entered a card has actually decided. Sign-ups without a card turn into no-shows more often.
Auto-charge has its own guide: Auto-Charging Maintenance Plan Visits When the Plumber Taps Complete.
The Customer App Path
Not every member signs up from a link. Existing customers who already use the customer app can sign up for services and pricing, such as a yearly flush plan, right there. They can also update their card later without calling the office, which saves a lot of declined-card follow-up when cards expire.
Website visitors who are not ready for a plan can use the request form instead. Those submissions also land in Form Responses, so the office has one place to check every morning. A request for a one-time flush is a good moment to mention the yearly plan, since the customer has already shown they care about the water heater.
Writing a Form People Finish
A few practical tips:
- Say what they get. At the top, list what the yearly visit includes: flush, anode rod check, T&P valve test, pressure check and a written report.
- Say the price. Customers distrust forms that hide it. “$129 per year, charged after each visit” is clear.
- Say when they will be charged. If the card is charged at completion, say so. Nobody likes a surprise charge.
- Say what happens next. “We'll text you to book your first visit” tells them the ball is in your court.
Close the Loop Quickly
A sign-up sitting in Form Responses for a week is a member cooling off. Convert new sign-ups daily and send a quick text confirming the plan and offering a first visit date. That one message turns a form submission into a relationship. To see how sign-ups feed the Waiting List, the inspection checklist and billing, go back to the water heater service software page.
